Nogami Ryoutarou adalah pemuda yang selalu apes. Suatu hari ia menemukan benda aneh (berupa tiket) dari seorang gadis misterius dan kereta besar yang mampu mengarungi waktu serta selanjutnya ia akan dirasuki suatu makhluk yang bernama Imagin (makhluk yang berasal dari masa depan yang kembali ke masa lalu untuk mengacaukan sejarah). Ryoutarou bersama Imagin Tukang Ngomel, Momotaros mampu berubah menjadi Kamen Rider Den-O yang bepergian mengarungi waktu dengan menggunakan kereta DenLiner untuk bertarung melawan Imagin jahat yang mencoba mengacaukan masa lalu yang berimbas pada masa sekarang dan depan. Selama petualangannya, Ryoutarou bertemu dengan Imagin baik yang kelak akan membantunya seperti Urataros (imagin bencong), Kintaros (imagin tukang tidur), dan Ryuutaros (imagin bocah). Selanjutnya ia bertemu dengan Sakurai Yuuto yang misterius bersama imagin setianya, Deneb. Yuuto tidak hanya seorang Kamen Rider Zeronos tapi merupakan "inkarnasi" dari tunangannya kakak perempuan Ryoutarou, Nogami Airi.
